1. A current NDIS plan - with plan management services included.
2. Details of your providers - copies of your current service agreements and/or the names of providers you’d like to use. If you are sure what service providers to use, we can suggest some for you.
3. Any updates or changes to your plan.
No. Plan management is your right, and it is free to you. Ask your NDIA planner or local area coordinator ('LAC') to set up your plan as “plan managed” and include “Improved Life Choices” in your budget at your initial planning meeting or review meeting. If your plan has already started, you can contact the NDIS to ask for a soft touch review.
Your National Disability Insurance Scheme ('NDIS') plan is all about you and your life goals. It is based on the details you provide in your planning meeting with your Local Area Coordinator ('LAC'). Your plan contains a breakdown of the different supports needed to help you achieve your goals, along with how much funding has been allocated to each category.
If you are mid-way through your plan and you are agency managed or self managed, but you want to make the switch to plan management, you will have to contact your NDIA planner or local area coordinator ('LAC') and request they do a ‘soft touch’ review to add plan management to your NDIS plan.
